About Reach Out Hertfordshire – North Herts Centre for Voluntary Services (NHCVS)
Reach Out is a project that aims to reduce loneliness in those who have recently been discharged from hospital, are are living with frailty by linking them up with a volunteer in their neighbourhood who can offer companionship and practical support to help them to feel less lonely and isolated.
