The Award requires you to demonstrate evidence of indicators linked to each theme in this checklist. This might be a policy, procedure, process, report, letter, email, or example from practice. Tick the box if you feel able that you could provide evidence that you have this in place.

- We don’t expect you to have everything in place, so please be honest with yourself. The Quality Award will support you to improve and we will give you feedback throughout.
